All-on-6 Dental Implants: Contemporary Tooth Restoration

All-on-6 Dental Implants: Contemporary Tooth Restoration

All-on-6 dental implants are an advanced technique for replacing a full set of teeth when a patient has experienced significant tooth loss. This innovative surgical solution involves the precise placement of six dental titanium implants in strategic positions and angles within the upper or lower jaw. These implants serve as a solid foundation for supporting a fixed full bridge, resulting in a natural-looking, stable, and durable restoration that remains permanently fixed in place. With All-on-6 implants, patients can enjoy the functionality and bite quality they would expect from natural teeth.

One of the key advantages of All-on-6 dental implants is their ability to address bone tissue loss without the need for additional procedures such as bone build-up or sinus lifting. This technique provides a suitable treatment option for patients who have lost most, if not all, of their teeth and struggle with loose dentures. By replacing the entire tooth set with just six implants, All-on-6 offers a reliable and long-lasting solution that restores both aesthetics and functionality.

Indications for All-on-6 Implantation:

  • Missing teeth accompanied by signs of bone atrophy (reduced bone height) due to insufficient chewing load in specific areas of the jaw.
  • Inability to install bridges on remaining teeth due to excessive dental mobility or abutment destruction, or when conservative treatment methods are ineffective.
  • Contraindications for bone build-up procedures or patient preference to avoid bone grafting.
  • Specific dietary preferences that involve consuming solid foods that exert significant load on the implants.
  • Diagnosed periodontal diseases.
  • Concerns regarding prosthesis stability when fewer abutments are used, as determined by the treating doctor.

All-on-6 dental implants offer a comprehensive tooth restoration solution for patients with specific indications, providing stability, durability, and an improved quality of life.

Benefits of All-On-6 Dental Implants

  • Natural look and feel

The All-on-6 dental implant system is meticulously customized to enhance your facial structure and provide you with the most attractive and natural smile of your life. It closely mimics the appearance and functionality of your natural teeth.

  • Enjoy a varied diet

Designed with convenience and stability in mind, All-on-6 oral implants do not restrict your diet like conventional dentures. Whether it’s sticky sweets, hard nuts, or hot soup, you can continue enjoying all your favorite foods without limitations.

  • Easy to maintain and clean

  • Prevents bone loss

The oral implants used in the All-on-6 system act as replacements for your natural tooth roots, stimulating your jawbone and preventing bone loss over time. Unlike conventional dentures, All-on-6 implants help maintain your bone structure, preventing the gradual loss of bone and the development of wrinkles.

  • Long-lasting solution

With proper dental care and maintenance, All-on-6 dental implants can last a significant portion of your lifetime. You won’t need frequent replacements as you would with dentures, providing you with a durable and reliable tooth restoration option.

  • Time and cost savings

By choosing All-on-6 dental implants, you make a one-time investment of both money and time. This approach saves you from spending excessive time in the dental chair for frequent adjustments or replacements. The efficiency of All-on-6 implants saves you both time and money in the long run, recognizing that time is valuable.

Advantages of All-on-6 Dental Implants in Turkey

  • Rapid results: Get new teeth in just one day. Fixed dental constructions can be immediately placed after the implants are inserted. These artificial teeth closely resemble natural teeth in both appearance and functionality.
  • Avoidance of bone augmentation: Osteoplasty is not required as the implantation is performed in areas with minimal bone resorption. The special design of titanium pins allows for their placement without the need for preliminary dental ridge augmentation.
  • Effective solution for total tooth loss: The all-on-6 protocol is a successful treatment option even in cases of severe bone atrophy and proximity to the maxillary sinuses. In complex situations, specially designed implants can be used, anchored at an angle through the orbital bone without affecting the adjacent maxillary sinuses.
  • Enhanced stability: The increased number of abutments in comparison to the all-on-4 technique improves the overall reliability of the dental construction, making it more resistant to different types of chewing forces.
  • Superior aesthetics: During the implant healing period, patients receive a temporary metal-framed plastic prosthesis. The smile appears flawless thanks to meticulously shaped gum lines and carefully selected parameters for the artificial teeth.
  • Restoration of chewing function: Patients can begin eating with their new prosthesis on the same day as the procedure (gradual loading of the implants is recommended). The overall functionality of the new teeth can be assessed once the implants have successfully integrated with the jawbone.

Difference between All-on-4 and All-on-6 Methods

The All-on-6 implant technique represents an advancement over the All-on-4 implantation technology, offering increased reliability and stability of the fixed dental construction. It is considered the cutting-edge solution for treating total tooth loss, providing patients with a new fixed prosthesis on the same day as the implant placement, allowing for immediate eating and smiling. Both the All-on-4 and All-on-6 protocols are one-phase implantations with immediate loading.

The All-on-6 protocol ensures a more balanced distribution of the chewing load on the implant system, reducing the risk of failure and preventing atrophic changes in the jaw. The procedure is similar to that of the All-on-4, but the All-on-6 treatment achieves higher prosthesis stability by adding two additional implants in the side areas of the jaw (one implant on each side).

The placement of the six artificial tooth roots is performed in three phases:

Two implants are placed in the front area of the jaw, aligned with the roots of the front incisors.

  • Two implants are placed in the side areas, mostly in a vertical position.
  • The remaining two implants are placed at an angle, depending on the condition of the bone tissue in the side areas (density, height, degree of atrophy).
  • The angle of inclination of the implants is compensated by a high-tech multi-unit abutment, which allows for vertical crown placement regardless of the implant’s position.

The Turkish All-on-6 method relies on precise calculations to determine the optimal locations for titanium implant placement, ensuring an even distribution of the load. The load is distributed trapezoidally, improving the stability of the abutments and making the entire system highly reliable.

Stages of All-on-6 Implantation

  1. Preparatory stage: During the initial visit, the patient undergoes consultation to determine the suitability of tooth implantation and a CT scan of the jaws to assess the feasibility of the All-on-6 technique.
  2. 3D modeling on a computer: Thorough calculation and computer modeling are performed to determine the optimal position and parameters of the implants based on the topography of the surrounding anatomical structures, especially in cases of significant bone tissue atrophy.
  3. Implantation: On a scheduled day, implants are placed (tooth extraction may also be required if necessary) using local anesthesia. The procedure takes several hours, and stitches are placed on the gum after implantation. Dental impressions are taken to create a temporary fixed prosthesis.
  4. Prosthesis placement: One of the notable advantages of this technique is the immediate placement of temporary prostheses. These temporary prostheses, consisting of a metal bar with plastic dental units, allow patients to chew and smile normally during the healing period of the implants. After 6-8 months (up to a maximum of 5 years), the temporary prosthesis is replaced with a final restoration made from suitable materials like metal ceramics or zirconium dioxide.
